6. Four (4) passport size photos

7. Copy of any government identification ( NIS, TRN, ID)

8. Copy of Birth Certificate 

9. Copy of Qualifications ( CSEC SUBJECTS, CERTIFICATES, DIPLOMA ETC)

Embassy Visa Application Fee $30,000

11. Valid Passport

12. General Medical Examination from any certified Doctor.
